Camp Forms
When you register your child or group, there are some necessary forms that must be filled out in order for your child/group to be received at the camp. Please download and print the following forms and have them available with you when you arrive at camp.

If you have registered your child or group, please fill out a Medical Release Form. You can get a Medical Release Form by clicking here.

If you have volunteers that are coming with your group, they must receive a background check. Please click here to get the form.

If you are interested in serving as a part of the Trinity Youth Camp staff, please click here to fill out an application.
